utilize their clinical skills. We provide training and
certification as needed in hospital-based practices.
Responsible for the interpretation, evaluation, preparation and
dispensing of medications in accordance with established policies,
procedures, and standards of practice of pharmacy services.
Collaborates with multidisciplinary care providers to monitor,
assess, and intervene as needed to ensure safe, appropriate, and
cost-effective medication use, with consideration of the aging
processes and human developmental stages (e.g., neonate, child,
adolescent, adult, geriatric). Applies pharmacotherapeutic and
pharmacokinetic principles to ensure safe and effective use of
medications for the patient populations served. Provides drug
information and consultation to providers, nurses, and other health
care professionals regarding appropriate drug therapy, including
drug interactions, side effects, incompatibilities, allergies,
medication order/reconciliation discrepancies, drug stability and
drug administration. Integrates patient-focused medication therapy
management and pharmaceutical care in accordance with established
clinical pathways or guidelines. Participates in prospective and
retrospective pharmacokinetic dosing and monitoring of selected
medications and recommends dose adjustments and laboratory
monitoring as needed.
Attends and participates in daily medical, multi-disciplinary and
specialty rounds and responds to emergency code blue situations.
Reviews and approves formulations for sterile products requiring
compounding or manipulation prior to dispensing and provides
in-process checks for medications prepared by pharmacy technicians
ensuring accuracy, sterility, and stability of compounded
products.
Participates in education and training of health care
practitioners, including pharmacy residents, students, interns,
technicians, nurses, and other health care professionals.
Participates in continuous quality improvement activities and
quality management initiatives including but not limited to adverse
drug reaction, clinical intervention, and medication incident
reporting programs.
*This position requires:*
- Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from
an accredited college of pharmacy.
- Prior to start date, possesses current unrestricted licensure as
a licensed pharmacist or as a licensed 90-day temporary pharmacist
in the State of Rhode Island. Obtains full licensure prior to
expiration of 90-day temporary pharmacist license, then possesses
and maintains current unrestricted licensure as licensed pharmacist
in the State of Rhode Island.
